Immigration Physicals (USCIS Form I-693)

As a USCIS-designated Civil Surgeon, Dr. Raja provides the medical exam required for your immigration application.

Services Included:

  • USCIS immigration medical exam

  • Completion & signing of Form I-693

  • Review of vaccination records

  • Tuberculosis (TB) testing

  • Required labs included

  • Guidance on any necessary vaccinations

Visitor Program

For visitors and guests to the U.S. who need outpatient care during their stay, Anchor Primary Care offers a one-time visit with a single follow-up. This program provides a simple, transparent option for short-term care when you are far from home. 

Services Included:

  • 45-minute in-person visit

  • In-person or by telehealth

  • One follow-up within 4 weeks

Scope of Care

Anchor Primary Care is not insurance. We are committed to providing comprehensive, evidence-based care within the scope of outpatient primary care and women's health—and part of caring for you well is being clear about what that includes, and what it doesn't.

There are some things we don't offer here. We don't manage chronic pain with opioids or prescribe chronic benzodiazepines for anxiety or sleep, and we don't provide long-term disability evaluations, workers' compensation care or DOT physicals. These needs are real and important, and they're often best served by clinicians who focus on them. When appropriate, we'll assist with referrals to specialists or community resources for care outside the scope of our practice.
